SIP Registration Forms for 2017 (submission deadline: Friday, May 26, 2017)

As part of the SIP registration process, all admitted interns and their parents/guardians must complete and submit the 2017 Registration Packet. This packet consists of the following forms:

1. Registration Documents Checklist
2. Intern and Parent Agreement Form
3. Elective/ Voluntary Activity Waiver Form
4. UCSC Intern Health Services Form
5. Health Facility Information Form
6. Copy of Medical Insurance Card (front & back; not included in the packet; needs to be submitted separately)
7. Internet Usage Agreement Form
8. Housing Agreement Form (*for only those interns who opt for campus housing)
9. UCSC Summer Handbook (please read but don’t submit this document)

Please click the link to download the 2017 SIP Registration Packet (fillable PDF) and complete/submit it by May 26 in one of the following ways: (1) complete/sign all forms electronically and email the packet to the SIP staff at ucsc-sip@ucsc.edu; (2) print the packet, complete/sign all the paper forms, scan them into a single PDF file, and email the file to the SIP staff; and (3) send the completed/signed paper forms by US mail to: Alexandria Leckliter, University of California Santa Cruz, Educational Partnership Center, Oakes College, 1156 High Street, Santa Cruz, CA 95064.

The registration fee payment (rest of the academic program fee and housing/charges) must be completed online at the Iris registration portal by the same May 26 date. Each intern should use the same login credentials (username = email address or login via Google, Facebook, etc.) that they used to pay the $60 application fee and $300 deposit; these login credentials are specific to this Iris registration portal (the SIP application portal firstname.lastname username and password will not work at the registration portal).

The submission of the registration forms listed on this page, housing/shuttle sign ups, and payment at the Iris registration portal complete the registration process.

Please contact the SIP staff if you need an extension for the registration packet submission/registration fee payment. However, it is important to keep in mind that SIP interns will not be allowed to start their research project when the program starts on June 12 until the completed registration forms packet and payment have been submitted.

If, for some reason, you no longer plan to participate in the 2017 SIP program, please let us know ASAP so we can assign another student to the research project.
